Home Care Program
Greystoke Homes & Support Services Inc. currently contracts with Chinook Health, CH (formerly CHR, Chinook Health Region) to provide In-Home Support Services (Home Care) for CH clients, as specified by CH on an individual basis. Caregiving services are provided to persons living within a designated are of the Chinook Health Region.
Historically, Chinook Health had four (4) contracted agencies providing home care in the Chinook Health Region. In January 2002 Chinook Health decreased to two (2) contracted agencies providing home care. Since May 1995, Greystoke has held successive home care contracts with Chinook Health.
Chinook Health is the primary funding source for the Greystoke Home Care Program. Greystoke Home Care Department, under the direction of Greystoke Owners, CEO and Executive Director, is responsible for hiring fornt-line staff and scheduling them to provide the required caregiving services, monitoring staff performance, monitoring quality of work performed, and attending to all other necessary tasks for the satisfactory fulfillment of all Greystoke obligations under CH contract.
Greystoke has a responsibility to provide hands on personal care as authorized by the Community Care Case Manager of Chinook Health. The community Care Case Manager authorizes the tasks/duties, frequency of service, and the duration of service. The Home Care Program primarily provides personal care services with minimal support services such as assistance with meals, laundry resulting from incontinence, and with respite care. Services are subject to change at short notice as changes occur in the needs of person served.
To effectively manage increasing needs, complex cases and high volumes, approximately 12,000 hours per month, the program currently employs one (1) Director of Reisdential Nursing, one (1) Assistant to the Director of Residential Nursing, four (4) Schedulers, one (1) Service Evaluator and an Education/Nursing Coordinator.
Many changes have occured within the home care program over the years. Initally, home care provided a significant amount of homemaking and support services along with personal care. The Chinook Health Community Care Program now has a mission to assist citizens to achieve and maintain personal health, wellbeing, and independence in their homes. This mission has led to a decrease in support services and an increase in personal care. The home care program is an integral part of keeping people in their own homes and allowing more opportunities for community living.
